Community Nursing Services
Community Nursing is an NDIS service provided by MMT Health that offers specialised nursing care and support to individuals with disabilities in their community or home settings. This service is designed to address the healthcare needs of people with disabilities, promoting their health and well-being while ensuring they can remain in their preferred living environment.
MMT Health begins by conducting a comprehensive assessment of the individual's healthcare needs, abilities, and goals. This assessment involves input from the person with a disability, their family, healthcare professionals, and the MMT Health team. This thorough assessment ensures that the care plan is tailored to the individual's specific support requirements.
Recognizing the need for specialized care, MMT Health employs highly qualified staff, including registered nurses, who have the necessary qualifications and experience to provide specialized care tailored to the individual's needs. Continuous training and competency updates are integral to our approach to ensure that our staff remains up-to-date with the latest healthcare practices.
Our approach to Community Nursing Services is highly collaborative, addressing the unique healthcare needs and goals of each person. This may include assistance with activities such as medication management, wound care, health assessments, and any other specific healthcare tasks identified in the assessment.
MMT Health maintains an ongoing monitoring and review process to ensure the effectiveness of the support plan. Regular check-ins and assessments are conducted at predetermined intervals or in response to changes in the individual's health or circumstances, allowing for necessary adjustments to the care plan.
